From Data to Decisions: Empowering Citizens and Health Workers with Digital Tools

Across Europe, digital innovations are redefining how citizens and healthcare professionals engage with data – shifting from passive information to active participation, from isolated tools to connected ecosystems. The goal is not only to digitalise healthcare but to humanise it through smarter, data-driven solutions that build trust, improve understanding, and strengthen resilience.

 

Funded by the EU’s research and innovation funding and implemented by diverse partners from academia, industry, healthcare, and civil society, several pioneering projects are leading this transformation. The Global Health Connector is proud to collaborate in many of them, helping to connect ecosystems, amplify impact, and accelerate real-world adoption. Below is just a selection of initiatives that show how data and digital can empower both citizens and health workers.

 

Gravitate-Health is helping citizens access and understand medicine information through the G-Lens app, which provides personalised, reliable insights based on users’ health profiles and literacy levels. Instead of navigating lengthy leaflets or uncertain online sources, patients can now visualise and interpret the information that matters most to them – improving adherence, safety, and confidence in treatment decisions.

 

BEAMER takes a complementary approach by exploring why people follow or abandon treatments. Its B-COMPASS model identifies behavioural factors influencing adherence and uses them to design digital tools that adapt to individual motivations and contexts. By combining behavioural science with digital health data, BEAMER offers a pathway to more personalised and effective care.

 

xShare focuses on the right to control one’s health data. Its Yellow Button concept allows citizens to access, download, and share their health records securely across systems and borders, in line with the emerging European Health Data Space (EHDS). This patient-centred exchange model puts people at the centre of data flows, supporting transparency, interoperability, and cross-border continuity of care.

 

4P-CAN pioneers personalised cancer prevention through two key innovations: a 4P-CANcer algorithm and Living Labs. The algorithm identifies individuals at higher risk for cancer, taking into account national and cultural contexts in Central and Eastern Europe, while the Living Labs bring citizens, professionals, and policymakers together to co-create new prevention measures. Together, they transform cancer prevention into a collaborative, data-driven, and inclusive effort.

 

Finally, KEEPCARING turns the focus towards healthcare professionals, co-creating digital tools that help strengthen their wellbeing and resilience. In a sector facing growing stress and workforce shortages, KEEPCARING’s solutions aim to ensure that digital transformation also supports those delivering care, not just those receiving it.

 

Together, these projects show how data and digital can empower both citizens and professionals – promoting understanding, participation, and sustainability across Europe’s health systems.
